What You Can Do to Prevent Elder Abuse
Health Care Directive/Advanced Planning
Individuals should have a current Health Care Directive (HCD). A current HCD is a gift to your loved ones and helps you get the care you would choose, if you were able.

Making my Wishes Clear offers statements one can review to help clarify their health care wishes and and ensure their health care directive is as clear and usable as possible.
​
The Provider Orders for Life Sustaining Treatment (POLST) form is a portable medical order that can give patients with advanced serious illness the option to exercise increased control over the treatment they do and do not want to receive at the end of life. The POLST form is NOT the same as a Health Care Directive, but it may be an important component of advance planning in certain circumstances. Unlike a healthcare directive, a POLST form must be signed by a licensed provider to be valid.
